Every facility in the directory is filed at the Tier-1 network level. Hospital list compiled from Antidote Health Plan of Ohio's CMS-filed machine-readable provider directory for plan year 2026 (directory records last updated November 2025), retrieved October 2026. Facility types come from each facility's federal NPI registration. Hospitals are listed under the names they were filed, so a hospital you use may appear under a different name, or may not be in the network at all. Outpatient clinics, urgent care, surgery centers, dialysis and nursing facilities in the directory are not shown. Networks change during the plan year and may change for 2027. Always confirm your own hospital and doctors in Antidote's live provider directory before you enroll, or let us check for you.

Check your doctors too. The directory also lists thousands of individual physicians and specialists, and not every doctor at an in-network hospital is in the network. We will confirm your hospitals and doctors before you enroll, at no cost.

Frequently Asked Questions

Is Antidote Health real ACA insurance?

Yes. Antidote's individual and family plans are ACA-compliant HMO plans, and you may qualify for premium tax credits based on your income when you buy through the Marketplace.

What does virtual-first mean?

Routine care can start with a virtual visit, which is usually faster and cheaper than an office visit. You still get in-person care through the plan's network when you need it.

When can I enroll in an Antidote plan in Ohio?

During Open Enrollment, which starts November 1 for coverage the following year, or mid-year if you qualify for a Special Enrollment Period. Ask us which dates apply to you.

Where do I enroll in an Antidote plan in Ohio?

Ohio residents shop for ACA plans on the federal Marketplace at HealthCare.gov. We can enroll you for free through our licensed Ohio agents, and compare Antidote with the other Ohio carriers such as Anthem, Medical Mutual, CareSource, Oscar, Ambetter and Molina.

Will Antidote's 2027 Ohio plans and premiums be different?

Plans and rates change every year. Antidote Health Plan of Ohio has proposed an average rate increase of 15.8% for 2027 before premium subsidies, according to HealthInsurance.org. Your own price depends on your age, county and subsidy. We will confirm final 2027 Ohio plans and premiums before Open Enrollment and update this page.

What Antidote drug coverage looks like today in Arizona

Antidote files a separate drug list in each state, so we cannot tell you from this comparison how its Ohio formulary treats any one drug. What we can show you is how Antidote builds a formulary in a state where it also operates. We pulled all seven Arizona carriers' 2026 drug lists as filed with CMS and compared them drug by drug.

2026 Arizona carrierCovers OzempicPrior authorization required
Antidote HealthYesNo
Blue Cross Blue Shield of ArizonaYesYes
AmbetterYesYes
Oscar HealthYesYes
UnitedHealthcareYesYes
CignaYesYes
ImperialYesYes

The same pattern holds across the whole class. Rybelsus, Mounjaro and Trulicity are all covered by Antidote with no prior authorization, while every other Arizona carrier requires it on all three.

The honest trade-off: Antidote applies step therapy instead. That means you may need to try a preferred alternative first and have it not work for you. You are swapping an approval process your doctor has to fight for a treatment sequence you have to follow. For many people that is the better deal, but it is not nothing, and it is worth knowing before you enrol rather than at the pharmacy counter.

Two more places Antidote comes out ahead in the filings: it is the only Arizona carrier listing insulin glargine with no prior authorization, no step therapy and no quantity limit, and the only one listing Eliquis with no quantity limit.

Source: each Arizona issuer's machine-readable formulary filed with CMS for plan year 2026, retrieved August 2026. This is Arizona data, not a description of Ohio plans. Antidote's Ohio drug list is searchable on its website, and we will compare Ohio carriers the same way.

This page is general information, not an offer of insurance.
